Family Care Enrollment

GWAAR operates the Family Care Enrollment project to assist consumers in Milwaukee and Fond du Lac counties with questions and enrollment for Family Care—Wisconsin’s long-term care service system. Aging & Disability Resource Center (ADRC) staff work directly with consumers in these counties to help them understand their options under Family Care, assist them in the enrollment process if they choose to become a member, and explain their rights and responsibilities once enrolled. In addition, Enrollment Consultants present all options regarding the selection of other programs that consumers may want to consider. Consultants review whether a consumer will have any cost share, discuss caregiving situations and service plan creation, and other details about the transition to Family Care. For consumers who have unresolved questions or want to explore other options, enrollment consultants are able to direct them to the appropriate resources to find the answers they need.
