Things to know

  • This self-assessment should be sent electronically in MS Word to GWAAR by Wednesday, February 29, 2012. Do not fax/mail.
  • This self-assessment covers the entire calendar year of 2011.
  • All goal statements from your plans should be copied and pasted into the assessment form (new for this year).
  • This self-assessment must be reviewed and approved by your commission on agings/tribal councils prior to submittal. Documentation of that review should be forwarded to GWAAR as soon as practicable.
  • If an aging unit is out of compliance, a corrective action plan is required by April 2, 2012.

FAQ

Submission

  • What am I required to submit?
    • All aging units are required to submit the Budget Form as well as the Aging Unit Plan Statement Page and Goal Form.
    • If you have any amendments or goal changes, you are required to submit the two signed Signature Pages as well as the Aging Unit Plan Goal Form.
    • If you have materially changed the content of your original plan or have substantial organizational, staffing, or other changes, you are required to hold a public hearing and to submit minutes and other information from that hearing.
  • What is the due date to submit my required information?
    • The deadline to submit all required materials is Tuesday, November 1.
  • Who do I send my budget/statement page/other to?
    • All information should be submitted to Sarah Cowen, Operations Coordinator, via email.
  • Do I need signed signature pages if I am only submitting the budget and statement page?
    • No, you do not. Signature pages are only required if you are amending your plan, i.e., if anything is filled out in the 2012 Aging Unit Plan Goal Amendment(s) section of the 2012 Aging Unit Plan Statement Page and Goal Form. They are not only required if you have substantial changes.
  • Are minutes required if I am amending my plan?
    • Not necessarily. They are only required if you are required to hold a public hearing, i.e., have substantial changes. If an aging unit plans substantial organizational, staffing, or other changes, the sections of the plan describing the changes must be submitted. A public hearing is always required in these circumstances.

Amending

  • Do I need to amend my plan if I need to change dates on my goals?
    • No, you do not. Activities that the aging unit or area agency had planned to complete by the end of 2012 but were not completed, may be extended into 2012, and completed during that year, without submitting a formal amendment.

Budgets

  • What allocation levels should I be working with?
    • Prepare the amendment materials based on the figures in the estimated allocations table. If an aging unit received 2009 carryover as part of their 2011 allocation, those funds should not be included.

Public Hearing

  • How do I know if I am required to hold a public hearing?
    • Public hearings are not required for the 2012 amendment unless the aging unit has materially changed the content of the original plan or, if an aging unit plans substantial organizational, staffing, or other changes. 
    • As always, if you have any questions you may contact the Older Americans Act Consultant assigned to your county (Wendell Holt, GLITC, for the tribes) for this amendment process.
